On the Job Training Minimum Requirements Must be 18 years of
age.
High School Diploma/GED
Current certification in CPR and First Aid or ability to obtain
certification within 30 days of employment
Have a valid current Oregon Driver's License, or plan to obtain
within 30 days of employment.
Hold a driving record that permits coverage under the agency's
corporate auto liability policy.
Be available to travel to multiple locations, multiple varying
distances, by the prescribed start-time of the coverage shift.
Must be flexible to work a variety of schedules and shifts with
varying days off.
Be able to lift up to 65 pounds for a total of up to 130 pounds in
a two-person lift Preferred Qualification, Skills, Knowledge and
Abilities Ability to communicate effectively (written and oral) and
appropriately with verbal or non-verbal individuals.
Ability to follow RN, physician, dental, and health care
professional's direction for care for a multitude of
individuals.
Ability to gather data and document accurately in English in
written and electronic format.
Ability to complete and be competent in all core competencies.
Ability to be flexible and work in a fast-paced environment.
Ability to work a variety of locations and change locations
mid-shift, travel to various locations and even multiple locations
in one shift.
Ability to complete tasks on time and meet deadlines.
Ability to drive a full-size passenger van equipped with a
wheelchair lift.
Ability to maintain confidential information.

Ability to participate in in-services. Essential Position Functions
and Key Work Processes Under the supervision of Adult Resident
Services Supervisor and the daily direction and monitoring of the
Shift Coordinator. Performs the following essential functions:
Relate to individuals in a continuous, consistent, warm, caring,
age-appropriate manner.
Perform personal care activities with a multitude of individuals
including but not limited to: personal hygiene care, brushing
teeth, brushing hair, bathing/showering, toileting, changing adult
incontinence pads, feeding (oral and gastric tube-if prescribed),
dressing.
Perform other activities of daily living for adults.
Provide individual medication administration according to agency
policy as ordered and assigned for a multitude of individuals.
Document the progress of each individual using approved data
collection methods.
Respond appropriately to immediate or urgent or emergent individual
needs, such as injuries and illness and provide or obtain
assistance as needed.
Monitor the activities of individuals in order to assure the
individual's health and safety is maintained.
Perform household cleaning and maintenance tasks including but not
limited to: sweeping, mopping, vacuuming, dusting, cleaning
toilets/sinks, washing windows, washing dishes, washing/folding
laundry.
Facilitate and/or perform meal planning and meal preparation in
accordance with dietician orders.
Facilitate and/or perform shopping and purchasing activities.
Assist in the planning of recreational/entertainment activities and
accompany individuals during such activities.
Maintain accurate records of the expenditure of agency and
individual funds.
Assure the individuals' environment meets or exceeds established
standards for health and safety.
Participate in individual care plans.
Maintain confidentiality of all individual records and
information.
Adhere to county and state regulations.
Document significant incidents and other individual related
information on approved forms or in specified electronic
locations.
Wear required personal protective equipment and follow all
applicable health protocols.
Report all injuries to the supervisor by the end of the shift on
which they occur.
Report hazardous conditions or other safety and health concerns to
the supervisor by the end of the shift on which they are
observed.
Maintain a safe working environment and fully participate in
emergency evacuation drills.
Always use safe lifting techniques.
Assist in the orientation and training of other staff.
Attend and actively participate in agency and individual related
meetings and during in-service training opportunities.
Travel to various locations frequently, be available for frequent
schedule changes and be available for work when called by
Supervisor in a reasonable timeframe (not engaged-to-wait).
Perform other job-related duties as assigned.
