Reports To: OPERATIONS MANAGER

I. THE PROGRAM SUPERVISOR IS RESPONSIBLE FOR THE OVERSIGHT OF
DIRECT CARE OF RESIDENTS AT WALKER SUPPORT SERVICES
Program Oversight - Development and maintenance of each resident's
Individual Support Plan. - Collaborate with Operations Manager on
weekly schedules for assigned programs. - Receive and respond to
client complaints/requests in a timely manner. - Attend and
participate in routine liaisons with CDDP for monitoring, as
requested - Complete monthly summaries for clients within assigned
programs. - Attend and participate in ISP Team Meetings as
scheduled. - Coordinate client appointments and ensure there is
transportation for each appointment. - Oversee assigned programs'
inventories for the programs and the individuals.
Staff Management - Respond to staff scheduling conflicts for
assigned programs. - Be on-call for employee questions for assigned
programs. - Ensure staff are current on all required trainings and
documents. - Weekly Lead Staff meetings with the assigned programs.
- Quarterly staff meetings with the assigned programs. -
Semi-Annual employee evaluations for assigned programs. - Complete
disciplinary action forms for staff.
Administrative Duties - Review daily documentation for each
resident to ensure documentation is completed appropriately and
accurately. - Provide emergency notification for serious incidents.
Respond to emergency situations as applicable. - Review incident
reports and associated data tracking.
Medical Management - Schedule all medical appointments with
assistance from Lead Staff to ensure information is exchanged and
obtained from medical providers. Complete follow-ups from
appointments as appropriate. - Follow-up on appointments not
attended to ensure necessary tasks occur as ordered.
Health & Safety Management - Ensure the programs are compliant with
weekly menus that are compliant with OARs. - Complete a monthly
health and safety check for the assigned programs. - Maintain
vehicle maintenance to ensure health & safety for residents and
staff members. - Facilitate household maintenance is completed as
required in order to ensure health & safety for residents and staff
members.
Financial Management - Maintain and supervise household expense
tracking. - Maintain and supervise resident expense tracking as
indicated in each resident's Financial Plan.
V. QUALIFICATIONS AND BACKGROUND/EXPERIENCE - Must be at least 21
years of age - Must have a Bachelor's Degree and/or 5 years of
relevant experience in 24-hour Residential Care - Must have a
minimum of 3 years of management experience - Must be able to write
Individual Support Plans - Current Oregon Driver's License - First
Aid and CPR certification - DHS Fingerprint and Background Check
compliance - Able to read, write, understand, and communicate in
English - Computer literate (Word, Excel, Outlook, Internet) - Can
make a commitment to be meaningful to a resident's life - Can
participate in pre-service training about your new job - Respect
resident and families - Can accept the temporary nature of
residential placement, have flexibility and a sense of humor - Can
participate in training classes as scheduled - Can demonstrate that
you are a creative problem solver and have good listening skills -
Must not have any founded allegations of abuse of a resident/child,
etc.
