    Summary

    In keeping with our organization's tradition, we seek to enhance the quality of life and holistic growth of older persons by providing high quality service in a cheerful, timely manner and by communicating, coordinating, and cooperating with others to attain goals. The Caregiver or certified nursing assistant position provides quality care to residents in order to meet their physical, emotional and spiritual needs.

      Essential Functions:


      • Provides quality nursing care to residents in an environment that promotes their rights, dignity, freedom of choice and individuality as illustrated by the following:



        • Provides individualized attention, which encourages each resident's ability to maintain or attain the highest practical physical, mental and psychosocial well-being.
        • Is knowledgeable of the individualized care plan for residents, and provides support to the residents according to their care plan. Contributes to the care planning process by providing the charge nurse or other care planning staff with specific information and observations of the residents' needs and preferences.
        • Attends to the individual needs of residents as directed, which includes assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, prosthetic appliances, transferring, ambulation, and range of motion, communicating or other needs in keeping with the individuals' care requirements.
        • Engages residents in meaningful conversation, socialization, and activity while providing personal care assistance
        • Maintains the comfort, privacy, and dignity of residents in the delivery of services to them. Interacts with residents in a manner that displays warmth and promotes a caring environment.
        • Fully understands all aspects of residents' rights, including the right to be free of abuse and neglect. Is responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents' rights.
        • Supports and upholds all company policies and care philosophy.
        • Assists in maintaining a safe, neat and clean environment; reports environmental deficiencies to the charge nurse or other management such as lighting or equipment problems.
        • Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items according to established policy and participates in efforts to locate missing items.
        • Observes residents for changes in physical, emotional, mental, or behavioral condition, unusual symptoms, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurse or supervisor on duty.
        • Provides care that maintains each resident's skin integrity by changing incontinent residents, turning, and repositioning immobile residents as stated or directed in their care plan or by supervisory staff.
        • Answers residents' calls for assistance and call lights promptly and courteously.
        • Performs various tasks assigned by the charge nurse, including checking vital signs, weighing residents, or other duties as directed.
        • Lifts, transfers and transports residents, using proper body mechanics or lifting devices.
        • Makes resident beds, empties resident garbage, and tidies resident rooms daily and as needed.
        • Changes resident bed linens according to facility schedule and as needed.
        • Changes resident towels on bath days and as needed if soiled.
        • Assists with resident laundry per shift guidelines and as requested.
        • Maintains resident clothing and clothing closet in a clean, neat and orderly manner
        • Cleans dining room tables and chairs after meals and snacks.
        • Notifies Housekeeper of spills and soiled areas; assists with clean-up as needed. When no Housekeeper is on duty, cleans up spills and soils immediately.
        • Straightens chairs, pillows and decorative items in common areas as needed, and returns items to appropriate location when found out of place.
        • Assists in cleaning wheelchairs, walkers, and other resident equipment per facility cleaning schedule and as needed.
        • Assists with dusting, vacuuming, and other housekeeping duties as assigned.
        • Communicates and interacts effectively and professionally with the residents, visitors, families, coworkers and supervisors.
        • Participates in activities and programming functions as directed.
        • Assists in the dining room during resident meal times. Assists during snack times and special events as directed.
        • Completes all forms and documentation such as incident reports, change of condition reports, and behavior tracking, as requested, and in a timely manner.
        • Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials.
        • Walks to and from various locations of the unit while making rounds and transports residents within the facility.
        • Promotes a homelike environment for residents.
        • Encourages residents to participate in scheduled activities and outings, and assists with those activities as needed or requested.
        • Responds to inquiries relating to his or her particular area or to requests from residents, visitors and other personnel as requested.

          Other Duties

          * Attends all required staff meetings and in-service training sessions

          * Demonstrates a positive attitude and contributes to an atmosphere of team spirit and cooperation within the facility

          * Answers phones as needed, in a prompt and courteous manner

          * Promotes customer service and positive community relations by treating residents, families, other staff, and all visitors in a courteous, helpful and friendly manner

          * Assists with other duties as assigned

          Working Conditions:

          • Must be able to work full-time or part-time as agreed upon, on assigned shift, including weekend and holiday rotations.



            • Regular, reliable attendance required.

              Physical/Sensory Requirements:


              • Medium work -- Ability to exert 25-50 pounds of force occasionally, and/or 10-25 pounds of force frequently, and/or up to 10 pounds of force constantly to lift, carry, push, pull or otherwise move objects.
              • Must be able to see well enough to write and read, and to visually interact with residents, families, and staff.
              • Must be able to speak and hear well enough to verbally interact with residents, families and staff.
