Job Description Primary Care Physician (Family Medicine /
Internal Medicine) Community Health Clinics – Oregon Salary:
$233,552 – $304,731 annually (Approximately $8,982 – $11,720
biweekly) Schedule: Full-time, part-time, and other flexible

community health system is seeking board certified or board
eligible Family Medicine and Internal Medicine Physicians to join a
multidisciplinary care team. This is an opportunity to provide

Do As a Physician, you will provide comprehensive primary care
services with an emphasis on prevention, chronic disease
management, and whole-person care: - Diagnose and treat acute and
chronic medical conditions through comprehensive patient
assessments, physical exams, and diagnostic testing - Develop and
manage individualized treatment plans, including prescribing and
monitoring medications - Provide preventive care, screenings, and
health education to support long-term wellness - Establish
therapeutic relationships with a diverse patient population,
including refugees and underserved communities - Coordinate care
and make referrals to specialty and community resources as needed -
Utilize panel management tools and data to support proactive care
and population health initiatives - Collaborate closely with
behavioral health providers and interdisciplinary care teams -
Provide clinical consultation and guidance to Advanced Practice
Providers, nurses, and support staff - Participate in committees,
quality improvement efforts, and community-based initiatives -
Maintain accurate and timely documentation within the electronic
health record Practice Environment - Multiple clinic locations
across the Portland metro area - Diverse, multicultural, and
multilingual patient population - Approximately 40% of patients are
non-English speaking, with interpreter services widely available -
Integrated care model including behavioral health and care
coordination - Team-based, collaborative clinical environment

Qualifications - MD or DO from an accredited medical school -
Completion of at least one year of residency in a primary care
setting - Ability to obtain Oregon medical licensure and
credentialing approval - CPR/BLS certification - Board
Certification or Board Eligibility in Family Medicine or Internal
Medicine - Experience working in community health or public health
settings, preferred - Commitment to serving underserved populations
