Registered Nurse (RN) | Home Health Location: Roseburg, OR Agency: Cynet Health Pay: $2,838 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 4/6/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Roseburg, Oregon, 97470! Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase and OASIS within the last 6 months Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders based on agency policies and procedures. Responsible for the competent application of the nursing process. Supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits and periodic office meetings are required. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Each type of visit and activity has a different point value, with a target of 30 points per week. Contractors will be assigned between 5-10 patients per day, equating to approximately 6-8 points. No guaranteed hours are provided; however, the contractor may discuss floating to a nearby clinic if patient census is low. Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. First-time travelers are not accepted; recent specialty experience is required. Weekend requirements vary by location and profession. On-call requirements vary by location and profession. Must be willing to travel up to 75 miles and mileage will be reimbursed at the IRS standard rate. Travelers are compensated from patient home to home. The commute to and from the first and last patient is not billable. Locals are accepted at the same rate. Pending licenses are not accepted; license must be in hand. Must be proficient in HomeCare HomeBase and Oasis, with experience within the last 6 months. Documentation must be completed within 24 hours of the patient appointment. No guaranteed hours are associated with this contract, and clinicians must meet weekly productivity points. All overtime must be pre-approved. Delayed and incomplete documentation that impacts billing and follow-up patient care may result in cancellation. All travelers must be open to on-call rotations.
