Job Description Description This position will travel to see
patients in the Portland Metro and Clark County (Washington) area.
This position will be working 32 hours/week with a start time of
1100 to 1500. The Hospice Access Field and Triage RN will complete
medical requirements while coordinating a whole-person plan of
care. Care is according to the individualized needs of the patient,
following the nursing process and in accordance with the Nurse
Practice Act. In Field role, the RN will visit patients in their
homes (private or facility) to present the hospice program and
conduct eligibility assessments, coordinating care with Hospice MD,
family and facility caregivers, and interdisciplinary team (IDT).
In the Triage role, RNs answer calls from patients and families
from the Providence Home Services building in NE Portland.

sufficient for licensure - Upon hire: Oregon Registered Nurse
License - Within 6 months (180 days) of hire: Washington Registered
Nurse License - Upon hire: National Provider BLS - American Heart
Association - Upon hire: In compliance with the Providence Health
and Services-OR, criminal background check is required for this
position, based on assigned duties. - 2 years of Acute care and/or
hospice, home health, or long-term care RN experience Preferred
Qualifications: - Bachelor's Degree in Nursing - Call Center
