Job Description:
Legacy Health is dedicated to improving the health of our

Washington. We are currently seeking a Nurse Practitioner for our
Outpatient Support at a Special Location, focused on delivering
high-quality, compassionate care in a team-oriented environment.
This role requires someone who can navigate complex patient cases
and enhance health outcomes while working closely with a
multidisciplinary team. Provide primary care and diagnostic
services to patients in an outpatient setting. Formulate treatment
plans and follow up on patient progress. Collaborate with
physicians, specialists, and other healthcare professionals to
ensure comprehensive patient care. Maintain accurate patient
records and prepare reports on patient status. Educate patients and
their families on treatments and healthcare maintenance.
Participate in continuous education and training sessions.
Implement new methods and technologies for improving patient care.
Strong diagnostic and decision-making skills. Excellent
interpersonal and communication skills. Ability to manage time and
prioritize tasks effectively. Competence in electronic medical
record (EMR) software. Medical Insurance Dental Insurance Vision
Insurance Life Insurance 401k Current Nurse Practitioner licensure
in the state of Oregon or Washington. Board certification in Family
Practice, Adult Practice, or another relevant specialty. BLS/ACLS
Certification.
