Registered Nurse (RN) | Case Manager Location: Hillsboro, OR Agency: Cynet Health Pay: $2,536 per week Shift Information: Days - 4 days x 10 hours Contract Duration: 13 Weeks Start Date: 2/23/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Hillsboro, Oregon, 97123! Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 10 hours (07:00 - 17:30) Experience: 1 year as an Inpatient Case Manager preferred; strong clinical nursing skills with five years hospital experience in varied clinical settings preferred. License: Current license as a Registered Nurse. Certifications: American Heart Association Healthcare Provider BLS. Must-Have: - Bachelor or Master of Science in Nursing. - Knowledge of insurance carriers and Medicare guidelines in the hospital environment. - Knowledge of community health and social service resources. - Experience coordinating complex discharges, care coordination, and arranging post-acute care services such as DME, Home Health, Hospice, Skilled Nursing Facilities, and Prior Authorizations for services. Description: Analyzes patient medical records concurrently and/or retrospectively for quality assessment and discharge planning criteria. Provides supportive documentation and clinical records to insurance carriers for authorization of services. Coordinates discharge plans and orders for patients leaving the hospital, including identifying needs for community services and making appropriate transition plans. This position requires creativity, problem solving, multi-tasking, and the ability to prioritize tasks and work with minimal supervision.
