Our team is passionate about making a difference in patients' lives through personalized services provided directly in their homes.Position OverviewWe are currently seeking a compassionate and experienced Registered Nurse (RN) to join our Salem-based Home Health team. In this role, you will assess patient needs, develop care plans, provide skilled nursing interventions, and educate patients and their families on managing their health.Key Responsibilities

• Conduct comprehensive initial and follow-up patient assessments.
• Develop and revise individualized care plans to reflect patient progress.
• Administer treatments and medications as prescribed.
• Provide patient and caregiver education to promote independence and recovery.
• Maintain clear communication with physicians and interdisciplinary care teams.
• Document patient care accurately and in a timely manner.
• Supervise Home Health Aides and evaluate their care delivery.
• Participate in on-call rotations as needed.Qualifications
  • Current Oregon RN license and BLS certification.
  • Graduate of an accredited nursing program.
  • Minimum of one year of recent acute care or home health experience preferred.
  • Bachelor's degree in Nursing is a plus.
  • Must possess a valid driver's license, reliable transportation, and insurance.Compensation
    • Hourly Rate: $55-$59/hour, negotiable based on years of service and home health experience.
    • Mileage Reimbursement provided for travel between patient homes.Employee Benefits
