Job Description:
TravelNurseSource is working with Cynet Health to find a
qualified Home Health RN in Salem, Oregon, 97301 About The Position
Summary : Home Health RN administers skilled nursing care to
patients on an intermittent basis in their place of residence. This
is performed in accordance with physician orders and plan of care
under the direction and supervision of the Director of Clinical
Services/Nursing Supervisor. Reports to Nurse Manager of Unit.
Duties and Responsibilities: Demonstrate knowledge of home health
regulatory standards and policies of agency. Complete a thorough
nursing assessment on admission. Initiates the plan of care for SOC
(Start of Care) using Medicare OASIS Form. Completed ROC
(resumption of Care) using Medicare OASIS Form Completes RECERT
(Recertification of Care) using Medicare OASIS Form Completes DC
(Discharge) using Medicare OASIS Form Performs initial evaluation
on patient visit, evaluates patient homebound status and re
evaluates the patient---s nursing needs on succeeding visits.
Provide services or treatments requiring substantial and
specialized nursing skill according with the written patient care
plan and the signed plan of treatment. Case Manages all therapies:
LPN, PT, OT, ST and CNA. Prepares clinical and progress notes for
each patient visit and summaries of care conferences on his/her
patients in a timely manner as per Agency policy. All paperwork to
be turned in over 24 hour period to assure Medicare Billing.
Counsels the patient and family/significant others in meeting
nursing and related needs. Processes orders and notifies physician
of patient needs and changes in condition. Conducts patient care
conferences on patient assigned to his/her care. Is available for
on-call duty nights, weekends and holidays, as assigned. Education
& Experience Requirements Graduate of an accredited school of
nursing License in good standing One year of recent Experience in
