spend quality-time caring for a smaller number of patients, giving
all patients the space, respect, compassion and care they
deserve.--At Landmark, our interdisciplinary teams collaboratively
manage our complex patient panels. These teams are led by
Physicians, Nurse Practitioners, and Physician Assistants, with
supporting care provided by RN Nurse Care Managers, Social Workers,
Pharmacists, Behavioral Health, and other employed team

communities.--The--Healthcare Ambassador--is a pivotal member of
the Landmark Interdisciplinary Team. This role actively engages
with patients both in the home and by telephone. The incumbent will
use compassion and empathy to earn patients' and caregivers' trust,
persistence to support and advocate for them, and excellent
communication skills to build impactful relationships.--This
position is full-time, Monday - Friday. Employees are required to
have flexibility to work any of our 8-hour shift schedules during
our normal business hours of 8:30am - 5:00pm. It may be necessary,
given the business need, to work occasional overtime.Primary
Responsibilities:
- Conduct home visits and/or phone or video calls to patients to
collect clinical and psychosocial information, and carry out
designated tasks, including but not limited to:----
- Resource referral and connection--
- Quality measure documentation, support, and follow up--
- Telemedicine facilitation and set up--
- Collect information regarding patient conditions using
standardized checklists and templates.--
- Partner with Landmark interdisciplinary team (providers,
nurses, social workers, etc.) to drive improvement in patient
outcomes and satisfaction--
- Actively participate in weekly interdisciplinary team
meetings--
- Reports directly to the Supervisor, Clinical
Nurse----Competencies:

- High School Diploma / GED (or higher)
- 2+ years of Costumer service experience
- 1+ years of clerical or office related experience
- Ability to travel up to 10% of the time
- Access to reliable transportation that will enable you to
travel to client and/or patient sites within a designated area
- Full COVID-19 vaccination is an essential job function of this
role. Candidates located in states that mandate COVID-19 booster
doses must also comply with those state requirements. UnitedHealth

well as all client requirements and will obtain necessary proof of
vaccination, and boosters when applicable, prior to employment to
