- On the Job TrainingGeneral Position SummaryThe LPN is primarily
responsible for providing LPN nursing care for people with
developmental disabilities. The LPN works under the direction of
the RN.Minimum Requirements
- Current license as a Licensed Practical Nurse in the state of
Oregon with no restrictions.
- High School Diploma/GED.
- Be able to lift up to 65 pounds for a total of up to 130 pounds
in a two-person lift.
- Current certification in CPR and First Aid or ability to obtain
certification within 30 days of hire.
- Reliable transportation, valid driver's license (or the ability
to obtain a valid Oregon driver license within 30 days of hire) and
a driving record that permits coverage under the agency's corporate
auto liability policy and personal automobile liability insurance
coverage.
- One to four years nursing experience preferred.
- Prior medically fragile and MR/DD experience preferred.
- Proof of COVID-19 vaccination.Preferred Qualifications, Skills,

- Knowledge and skill of LPN nursing.

- Under the direction of the supervising site RN and Residential
Program Director, ensures that all medical and related needs of
customers are met as directed through:
- Implementation of the medication and treatment administration
process.
- Implementation of Nursing Care Plans; to include monthly
Nursing Care Plan reviews and reporting of monthly Nursing Care
Plan outcomes.
- Accurate nursing documentation with a minimum of weekly nursing
notes on each customer.
- Maintain continuous observation of customer health needs and
report any changes/concerns immediately to the RN followed by
appropriate written documentation.
- Provide hands on treatment/nursing procedures.
- Implement nursing procedures as directed.
- Implementation of physician, dentist, and nursing orders.
- Documenting any changes in status, concerns, or information
from consultants and MD.
- Assist in maintenance of the Medical Administration Record for
each customer.
- Work under the clinical direction of the RN, or other licensed
provider who has the authority to make changes in the plan of care,
and applying practical nursing knowledge drawn from the biological,
psychological, social, sexual, economic, cultural and spiritual
aspects of the customer's condition or needs.
- Relate to customers in a continuous, consistent, warm, caring,
age-appropriate manner.
- Monitor the activities of customers in order to assure the
individual's health and safety is maintained.
- Respond appropriately to immediate or urgent or emergent
customer needs, such as injuries and illness and provide or obtain
assistance as needed.
- Participate in customer care plans.
- Maintain confidentiality of all customer records and
information.
- Adhere to county and state regulations.
- Document significant incidents and other customer related
information on approved forms or in specified electronic
locations.
- Wear required personal protective equipment.
- Report all injuries to the supervisor by the end of the shift
on which they occur.
- Report hazardous conditions or other safety and health concerns
to the supervisor by the end of the shift on which they are
observed.
- Maintain a safe working environment and fully participate in
emergency evacuation drills.
- Use safe lifting techniques at all times.
- Review and practice recommended health and safety
procedures.
- Attend and actively participate in agency and customer related
meetings and during in-service training opportunities.
- Provides guidance during customer emergency
situations.Secondary Position Functions
- Perform personal care activities with all customers as needed
including but not limited to: personal hygiene care, brushing
teeth, brushing hair, bathing/showering, toileting, changing adult
incontinence pads, feeding (oral and gastric tube-if prescribed),
dressing.
- Perform other activities of daily living for adults.
- Perform household cleaning and maintenance tasks including but
not limited to sweeping, mopping, vacuuming, dusting, cleaning
toilets/sinks, washing windows, washing dishes, washing/folding
laundry.
- Facilitate and/or perform meal planning and meal preparation in
accordance with dietician orders.
- Other duties as assigned by Registered Nurse and the Adult
Services Program Director.
