Salem Health Hospitals and Clinics complies with the Oregon

Description/Qualifications
Eligible for excellent hiring incentives, including a Start Bonus
and Relocation Assistance (for hires relocating from greater than
100 miles). To qualify for the bonus candidates must not have been
employed by Salem Health in the last 6 months. Bonuses are
available to external applicants only and are subject to all
applicable tax withholding.
Your Role With Us:
As the Cardiac RN Navigator, you will implement and refine patient
navigation services for cardiac patients throughout the continuum
of care. You'll provide expert individualized care coordination to
patients and their families, by connecting patients with resources
and support systems. You will also be responsible for facilitating
interaction and communication with providers and health care staff,
track interventions/outcomes and streamline appointments and
paperwork.
Core Functions:
* Provides care coordination to cardiac patients and their
families.
* Coordinates and executes program development.
* Assists leadership with department projects as assigned.
Background/Expertise:
* Requires BSN or non-nursing Bachelor's Degree with MSN for all
external applicants for Salem Hospital, preferred for West Valley
Hospital. For internal applicants preference may be given to
applicant with BSN.
* Minimum of three (3) years of experience in the health care field
with strong clinical background.
* Two (2) years clinical experience in cardiac nursing
preferred.
* Registered Nurse in the state of Oregon.
* BLS certification from the AHA required.
* ACLS certification required.
* ANCC Cardiac-Vascular, CCRN, or equivalent certification required
within two (2) years of hire.
