Job Description:
A company is looking for a Clinical Care Manager RN I (Health
Plans).
Key Responsibilities
Assess, plan, implement, coordinate, monitor, and evaluate health
services for individuals
Provide care management interventions and support across
transitions of care
Identify resources and eliminate barriers to meet health needs
Required Qualifications, Training, and Education
Current unencumbered Oregon RN License required; BSN preferred
One year of clinical nursing experience required
Experience or training in health care delivery systems and managed
care patients
Proficiency in computer applications, including electronic
documentation
Experience in case management and knowledge of Medicare and
Medicaid regulations preferred
